I agree to voluntarily participate in dance classes and activities offered by Columbus Salsa Classes LLC. I understand and acknowledge that participation in these activities involves risks of injury or harm. These risks include, but are not limited to, slips, falls, physical strain, collisions with other participants, and other injuries that may occur during dance classes or related activities. I hereby assume full responsibility for all such risks.

I acknowledge that I have been advised by Columbus Salsa Classes LLC to consult a physician before starting any exercise program and that I have no physical or medical condition that would prevent me from participating in these activities.

I agree to release, discharge, indemnify, and hold harmless Columbus Salsa Classes LLC, its employees, instructors, agents, representatives, volunteers, and landlords from any and all claims, liabilities, damages, and expenses (including attorney's fees and costs) arising from or related to my participation in the dance classes and activities, including but not limited to those caused by negligence or otherwise.

I understand that Columbus Salsa Classes LLC may occasionally photograph or video-record classes or events for promotional purposes. I grant Columbus Salsa Classes LLC the irrevocable right to use these images or recordings, which may include my likeness, without compensation or further approval.

If the participant is a minor, I affirm that I am the parent or legal guardian of the participant, and I agree to the terms of this waiver on their behalf.

I further agree that any dispute or claim arising out of or in connection with this waiver or my participation in the activities will be resolved exclusively through binding arbitration, in accordance with the rules of the American Arbitration Association.

This waiver is binding and enforceable. If any provision of this waiver is found to be invalid or unenforceable, the remaining provisions will remain in full force and effect.
